CASE STUDIES A Standing Complaint: Inability to Sit: An Unusual Presentation of Medial Hamstring Myofascial Pain Syndrome
نویسنده
چکیده
Background: Hamstring muscle injuries are among the most common lower extremity injuries in athletes. Pain in the posterior thigh and leg weakness are usual presenting symptoms. Less dramatic onset in nonathletes, without pain in the posterior thigh, is more often thought of as an ischial bursitis or as pain from the sacroiliac joint or the gluteal or piriformis muscles. Pain localized over the ischial tuberosity resulting in disabling discomfort when sitting as the only symptom of medial hamstring muscle trigger points [TrPs] has not been reported before. Six cases of hamstring muscle myofascial pain syndrome are reported with this unusual presentation. Findings: Two cases were only single muscle syndromes, and in four cases other regional muscles also had TrPs that were symptomatic at some time. Only the ischiocondylar head of the adductor magnus [a false Robert D. The diagnosis was supported by the finding of localized tenderness in the proximal medial hamstring muscles [and also in the adductor magnus in two cases], and confirmed by diagnostic TrP injection of local anesthetic into the muscle. In each case, TrPs were located in the proximal portion of the muscle. Treatment of the medial hamstring muscles [including the adductor magnus muscle in the one case] produced a complete resolution or significant improvement in all cases. Conclusion: Medial hamstring muscle myofascial pain syndrome may present as a predominantly single muscle syndrome. Pain can be confined to the region of the ischial tuberosity, the only complaint being the inability to sit. Localization of TrPs in the proximal portion of muscle, above the tendinous band inscription of the muscle, may be a factor in proximal pain referral. Eccentric contraction, muscle fatigue, and inflexibility are factors that predispose to hamstring injury. Purpose: To report the presence of hypertropia with esotropia in the case of anisometropic Heavy Eye Syndrome (HES). Case Report: A 33-years old lady referred to the strabismus clinic for evaluation. The patientchr('39')s esotropia started at the age of 12 and gradually increased. Corrected visual acuity was 2/10 and 4/100 and refraction was -24-3.25×103 and -10.25-0.75× 34 in the right and lef...
متن کاملReport of an unusual complication of radiofrequency neurotomy of medial branches of dorsal rami.
Radiofrequency neurotomy (RFN) of the medial branches (MB) of the lumbar dorsal rami is a common procedure to treat facetogenic backache. Serious complications are unusual when guidelines are followed (1). We report for the first time, the development of severely painful camptocormia (inability to straighten the back in the upright position that disappears in the recumbent position) as a compli...
متن کاملThe Effects of Ultrasound-Guided Dry Needling on Patients With Myofascial Pain Syndrome: A Review Study
Objectives: Dry Needling (DN) is a novel and effective intervention for patients with Myofascial Pain Syndrome (MPS). Some characteristics, such as needle penetration depth, needle insertion into the target muscle, and trigger points must be identified in this intervention. The Ultrasound (US)-guided DN is a technique that involves needle insertion at the site of injury and the simulation of ti...
متن کاملA prospective study evaluating the effects of MRT on the treatment of patellofemoral pain syndrome
INTRODUCTION Knee cap pain is the most common knee complaint seen in young adults. 1,2,3 Knee pain treatment research is vast. However little research has been conducted with a popular and relatively new treatment called the myofascial release technique (MRT). This treatment is a hands-on soft-tissue injury treatment utilized globally by many health care professionals. The objective of this stu...
متن کاملUnusual Presentation of Squamous Cell Carcinoma on Long-Standing Sacrococcygeal Pilonidal Sinus
Pilonidal disease consists of a hair-containing sinus or abscess occurring most frequently in intergluteal cleft. This disease is generally benign. Although very uncommon entity, it seems reasonable to be aware of possible malignancy in longstanding cases. We report a case of squamous cell carcinoma in a 52- year-old man, with a prolonged history of pilonidal disease and ulceration since 3 mont...
